Equipment to Be Set up by MIS Staff
Policy of Using Media Equipment
One-business day notice is required. We only deliver and install equipment for R-MC instructional functions thatrequire set-up or technical assistance. MIS does not support non-College functions, and will not deliver hand-carried equipment such as voice recorders or compact video cameras. Please come to the Library to check out such items.
AGREEMENT
(PLEASE READ CAREFULLY)
By filling out the form, you agree that you have made sure the following terms are understood by the Person in charge of the event for which the equipment will be set up.
- The person in charge of the event (the Person) for which the equipment will be set up will be responsible for the security of the equipment, from the time of set-up to the time of retrieval/return.
- The Person will be present when the equipment is being set up. The MIS staff will contact the Person on the day of the event for a specific set-up time. In the event he or she cannot be present, the Person must come to the Media & Instructional Support Office to sign the equipment checkout form, and provide the contact information of a designee who will receive the equipment upon its delivery. The MIS staff will not set up the equipment if nobody receives it.
- The Person will use the equipment properly and responsibly, and will contact the MIS staff for returning/retrieving the equipment.
If the above terms are acceptable to you and the Person, continue filling out the form to request the equipment be delivered and set up by the MIS staff.
If the above terms are unacceptable, you or the Person must come to MIS office to check out the equipment and bring it back after use. Please fill out the Equipment CHECK-OUT Form for that. |
