 Please click the link below to print a copy of the REQUIRED Health History Record pages.  These 4 pages must be completed by all new students entering Randolph-Macon College.  All 4 pages must be signed by your physician or health care provider.     FAXES WILL NOT BE ACCEPTED.

All students should carry health insurance.  Students should have a copy of their health insurance card and present it to the Health Center staff on each visit to the Center.

Randolph-Macon is currently reviewing its student health care insurance policy.  Updates will be posted as they become available.
