I,______________________________(print name), understand that while trying out and / or being a member of the Randolph-Macon College Dance Team, have read the team Mission Statement and Consent Form and agree to abide by these rules and regulations as set forth by the Randolph-Macon College Dance Team Mission Statement and the Randolph-Macon College Dance Team Consent Form.
As a member of the Dance Team, in the event that I sustain an injury (of any magnitude, including death) during my participation in dance activities, I will be legally and financially responsible for any bills related to the aforementioned injury.
I hereby release Randolph-Macon College, its facilities, the Randolph-Macon Athletic Department and its coaches, club coaches, and staff from any liability in the event of injury while participating in any instruction, activity or travel to and from a Dance event.
