The College’s group health insurance plan is administered through Anthem via CICV and the cost is shared by employees and the College.The plan contains a choice between a health maintenance organization(HMO) and a preferred provider(PPO) plan.

Please note that some Ophthalmologists are in the HMO Healthkeepers network or accept the PPO plan for medical services they provide, but do not necessarily participate in Blue View Vision plan.  This means that if you see such an Opthalmologist for a medical condition, the ophthalmologist may not accept your benefit for the routine eye exam service part of the exam.  For the routine eye exam done at an Opthalmologist at the in-network rate, they must be participants in the Blue View Vision plan and should be listed when you do the search of vision providers below if they accept Blue View Vision.  To be sure that a provider is in the Blue View network, check with Anthem Customer Service for your HMO or PPO Plan.  Their numbers are listed above.

If you go to a non-participating Ophthalmologist for a routine eye exam, you will need to file a claim yourself in order to the get the $30 allowance for your eye exam.  Claim forms are available on the anthem.com website:  http://www.anthem.com/shared/noapplication/f5/s1/t0/pw_m008894.pdf?refer=ahpmember&state=va
