Please list below the names of three R-MC faculty/staff who have agreed to serve as references. ALL applicants must include a reference from their academic advisor. In addition, applicants must include as referees instructors who can attest to their aptitude for the HAC positions(s) they are seeking. Three (3) references are required unless waived by a HAC administrator. In addition to listing references below, it is the applicant’s responsibility to request each referee to complete the on-line reference form found on the HAC web site.
